BENEFIT RENEWAL
The Ohio Department of Medicaid must recheck eligibility every 12 months. To renew your benefits,
Fill out the packet they send to your stated address
Go to “renew your benefits” on benefits.ohio.gov
Visit your county Job and Family Services office.
Remember to
check your insurance card for its expiration date
renew your benefits before this deadline
notify your Job and Family Services office if your address changes.
